Apple previewed watchOS 5 on June 4, 2018; its register dates release to September 17. Compatibility changed: Series 1 or later was required, excluding the original Apple Watch. The announcement required iPhone 5s or later with iOS 12, without creating new hardware catalog entries.

Activity added one-to-one seven-day competitions. Workout detection prompted users to begin with retroactive credit and reminded them to finish; it did not mean every activity was perfectly recorded without intervention. Yoga, hiking, cadence, target pace and rolling pace expanded Workout.

Walkie-Talkie offered push-to-talk over Wi-Fi or cellular, not independent radio between watches, with country restrictions. Podcasts could sync episodes for local listening or stream them. ECG arrived later in 5.1.2 for Series 4 in the US and US territories, not every watch or country at 5.0 launch.

Zack Whittaker reported in TechCrunch the withdrawal of 5.1 after startup failures and the release of 5.1.1 on November 5, 2018; no independent affected-user count is inferred. The 5.3 notes include a Walkie-Talkie security fix. Much later, 5.3.10, dated March 24, 2026, extended a certificate to keep listed services working after January 2027, not a new generation of features.
